How do I upload my patient form?

Use our built-in uploader to load your patient forms to the system. Start by clicking the "Browse" button. This will open a window that shows you the files on your computer. Simply select the file you want, and click "Open." You may select more than one file to upload at a time. When you have selected all the files you want to upload, just click the "Upload" button and they will be added to your account.

***Pay attention to your file size. Very large files (more than 1000KB) may take a long time to upload, and may also slow down your visitors as they use your site. You can check the size of a file by using the right-click button on your mouse and looking at the file properties.





What form should I upload?

It's entirely up to you which form you want to upload for your patients. You may only have one downloadable file for your patients at the World Eye Web. We recommend that you choose the form that patients most often use in the office, for instance, Health History.

However, if you combine your forms into one file, then you can efficiently provide all your pertinent forms in one download.





What kind of files can I use for patient forms?

The only important thing to remember is that your patients will need to be able to read the forms with their own computers. Use a common file type, such as MS Word, or Adobe PDF. Word offers a feature that allows you to create a PDF directly.





Can my patients download my form?

So long as your patients have the correct software to view your forms, they will be able to download them. To make this easy for patients, we recommend that you use a common software to create the forms, for instance, MS Word or Adobe PDF formats are generally readable by most computer users.

To download your forms, your visitor will just need to click on your "Download Forms" button, and the computer will do the rest.





Why should I bother to upload patient forms?

Your patients are becoming more web-savvy every day. Many people exclusively use doctors, vendors, and stores that they can access from the Internet. Offering your patient forms for download lets your patients save valuable time in the office, and can make their visit more pleasant. Additionally, it saves staff and chair time if your patients are not slowed down by paperwork when they first arrive for their appointments.





What if I don't have patient forms available in digital format?

It's pretty easy to get your forms converted to digital. You can create the form again using a program such as MS Word, or have the forms converted by a document scanning service. Ask your local print-shop, or business-supply store about scanning your documents. They will be able to create a usable PDF from a paper copy for a nominal fee.
